Health Information

Thomas MacLaren School is committed to your child’s health and wellness.  Our staff is available to respond to emergencies, assess minor bumps and bruises, and evaluate students who feel ill, supporting those who need treatment and/or rest, or sending them home, if needed. 

Parents are encouraged to assess the health of their child every day before they come to school. Please use the following guidance to help inform your decision whether your child should come to school or not. Thank you for helping keep our community sickness-free!

Illness Guidelines: Please use this information to decide whether or not your child should stay home from school when they feel ill.

To help us better care for each student, we ask parents/guardians to submit an Annual Health Update, including latest vaccination records or exemptions, and Health Care Plans (see below), before the start of each school year.
  • The Annual Health Update form is part of the electronic Student Update packet that is emailed to every family to fill out as a part of the yearly Registration process. 
  • Vaccination records/exemptions can be uploaded as part of the online process.  We will accept records from your health provider's office or from your previous school. Vaccination records/exemptions that are not uploaded at registration time can be submitted in the following ways:
    • Faxed using our Secure Fax number - (866) 587-2608, OR
    • Hard copy mailed or hand-delivered to the school (Attention: School Nurse)
  • If you are seeking an exemption, please go to the Colorado state website page regarding Vaccine Exemptions to read the requirements and download the applicable forms to submit.
  • Paying for vaccinations
    • If you need help finding free or low-cost vaccines and health care providers who offer this service, you may:

Medication at School: For any student requiring medication during the school day, the Authorization for the Administration of Medication by School Personnel must be completed before any medications can be administered. This form must accompany EACH medication, INCLUDING over-the-counter medication. It must be completed by the parent/guardian AND signed by a health care provider with prescriptive authority.

Students with Severe Food Allergies or Intolerances: If your child has severe food allergies or intolerances, please read these important guidelines that we use at MacLaren. Heath Plan Information: Our priority in the health care room is to care for your child.  If your child has

  • Allergies,
  • Asthma,
  • Celiac Disease,
  • Diabetes,
  • Migraines,
  • Seizures, or a
  • General Health Care Issue, such as
    • Autism
    • Bleeding disorders
    • Cancer
    • Cerebral Palsy
    • Digestive disorders
    • Down Syndrome
    • Head Injury
    • Heart Condition
    • Hydrocephalus/Shunt
    • Hypoglycemia
    • Hearing Loss
    • Muscular Dystrophy
    • Orthopedic Problem
    • Severe Skin Disorder
    • Spina Bifida
    • Severe Visual Disorder
    • Any other severe, diagnosed health / emotional concern/condition
    • Academic Accommodations (for a general health diagnosis)

then the parent/guardian must submit, before the first day of school, the appropriate health care plan (see below), filled out completely (including signatures) by the parent/guardian and health care professional with prescriptive authority. Forms may be submitted via fax using our secure fax line, (866) 587-2608, or by delivering them to the school in person, or by mail. Please mark them to the attention of School Nurse.

The health care plans below contain all necessary documents to allow your child to receive medicine at school and self-carry inhalers or Epi-Pens®, if desired.
